Including your Etudely version, Chrome and OS, what happened, steps to reproduce, and the expected result helps us investigate.

Do not send sensitive information

Do not enter passwords, authentication codes, card numbers, payment information, or other sensitive information. File uploads are not accepted.

The form uses Google Forms. Google sign-in is not required, and you can submit anonymously when you do not need a reply.
